The form below will ask you questions about your faith journey (including your baptism) and will require you to agree to our Statement of Accountability. Each person applying for partnership needs to complete all parts of the application. (i.e. A husband and wife desiring partnership need to complete two separate applications.) Questions or statements with a * following them must be completed before submitting the application.

At the end of the application we will ask you to agree to our Statement of Accountability. By submitting this form you agree to this statement: As a partner of LifeBridge Church, I agree to support this ministry with my time, my gifts and talents, and my finances. I will also be accountable to the church leadership in matters concerning my relationship with God and with others.

Our mission at LifeBridge Church is to connect people with God and other people. This first section about your spiritual history helps us to learn about your connection to God through Jesus Christ. Please be as complete and concise as possible.

The Bible teaches that God is relational. He desires a growing relationship with everyone. However, we are born separated from God because of sin. God's desire for a relationship with us is so great that He sent His only Son, Jesus Christ, to die on a cross as the payment for our sin. All who believe that Jesus Christ died on their behalf and believe that His death paid for their sin have a relationship with God.
